You will need the Bill Pay Code and Amount Due from a recent statement.

Insurance

We accept payment from many medical insurance plans. If we are a participating provider on your plan, we will accept your valid insurance card as payment for the services we provide you. If a co-payment, deductible, or co-insurance applies, you are required to pay that at the time of service.

We will also file a claim on your behalf directly to your insurance company. Since many people’s insurance benefits (including co-payment, deductible, and co-insurance amounts), billing addresses and eligibility change frequently, we rely on the information printed on your insurance card to properly file your claim.

If your insurance plan requires a referral for you to see us, you are responsible for obtaining that referral and providing that information to us prior to your appointment.
